The Connected Care Network (CCN) is a point of access for children who you are working with where a professional feels that there is an unmet need or a difficulty finding the right service for the child. All referrals can go through the above links (NB: No urgent referrals should be made to CCN – Please use existing pathways) and a health and wellbeing co-ordinator will triage it and direct to the appropriate service(s).
We have access to a unique array of services available ONLY to the Connected Care Network as well using existing services across schools/education, services commissioned by the NHS and Solihull Council as well the voluntary and community sector; providing a basis for true integrated working and throughout the process there is clinical support and supervision.
We currently have no other referral criteria other than there has to be a child involved who has a need! All we ask is that where a professional feels they are unable to address a need that they have identified for a child and family, they should refer to CCN.
Examples of cases we have helped to manage – children struggling to attend school, children on a waiting list for mental health support, children on a waiting list for autism assessment, children suffering with urinary and bowel problems, parents concerned about their child’s sleep and many more.
You as the referrer will get direct feedback via the system as to any / all interventions that take place so that records are kept up to date.
