This referral form is for professionals only
If you are a parent, please speak to a professional that is involved with working with your child to make a referral
Please choose from one of the links above to make a referral to the Connected Care Network; it is imperative that you have gained consent and informed the main carer of the child/young person that you are doing the referral

One of our Health and Wellbeing Co-ordinators will call the family/carer and find out more about the needs of the child/young person you have referred.

We will use our ‘Tell The Story Once’ template and Multidisciplinary Team to create a plan of action that is bespoke to the needs of the child/young person/family.

We will do onward referrals, and signposting, to local services, agencies and organisations with the ‘Tell The Story Once’ template being forwarded when necessary, so that the family do not have to tell the story again! The referrer will also be informed of any onward referrals / signposting

We will follow up with the family/carer and any onward referrals made within three months to ensure that progress is being made.
