1 – Follow the link to make a referral to the Connected Care Network

2 – Wait for one of our Health Care Navigator to call and find out more about your child.  We want to know who they are in a very holistic way, finding out about their sleep, diet, social skills, education, what they enjoy and how they maybe struggling. A Tell The Story Once form is created which will have the referral information and the transcript from triage the telephone call included so that you do not have to keep repeating yourself.

3 – The Tell The Story Once form will be presented to a small team of Health Care Navigators and the Clinical Lead to ensure that the plan of action is bespoke to the needs of your child.

4 – Referrals to local services, agencies and organisations will be sent out on your behalf with the Tell The Story Once being forwarded so that you do not have to tell the story again!   Additional support and information will also be signposted to you if appropriate.

5 – Our partner local organisations will get in touch when a referral has been made to them and invite your child to join in the appropriate activities, or make appointments for therapies, liaise with school, refer to or check existing referrals to NHS services,  if appropriate for your child’s care plan. Follow up with yourself and with the organisations will be made in three months time to ensure that progress is being made.
